About the Role

The Engineering Manager, Messaging & Applications owns and drives the technical direction of the two teams responsible for CareMessage's core product. The Messaging team builds the systems that carry every patient conversation. The Applications team builds the web application that providers use to run those conversations. CareMessage supports over 400 safety-net healthcare organizations, including federally qualified health centers and free clinics that care for low-income and underserved patients nationwide, and these two teams build what those patients and their care teams actually touch. When a patient receives an appointment reminder at the right moment, in the right language, on a phone that may be their only connection to the health system, that is your teams' work.

This is a leadership role over existing teams. Their systems and roadmaps are already in place, and your job is to lead, deepen, and scale them. Both teams work in the same core codebase, so a core part of this role is holding clear ownership between them without slowing either one down.

The Messaging team builds and maintains the systems that drive all outbound and inbound patient communication across SMS, voice, and multimedia, powered by Twilio, with active investment in AI-driven message categorization, MMS/RCS, intelligent delivery optimization, and consent management. The Applications team builds and maintains the core web application, a Ruby on Rails monolith with a JavaScript/TypeScript/React front end on Postgres and GCP, through which providers manage campaigns, patients, appointments, and messaging. That experience is increasingly enhanced by our solution recommendation engine and by AI copilots that help providers determine and act on the right next step for each patient. In this role you will work closely with Product, Design, Customer Success, Compliance, and the rest of Engineering to design and deliver the messaging and application features that deepen patient engagement and health outcomes, with direct ownership of technical decisions and customer-facing system behavior.

What You Will Do

A. Lead and develop two engineering teams to prioritize driving health equity outcomes and impact over activities

  • Own hiring, feedback, performance, and career growth for the engineers across the Messaging and Applications teams, half of whom are early in their careers

  • Set the operating cadence for both teams, including planning, retrospectives, and tech debt reviews

  • Set the performance bar for both teams and hold it, with specific feedback in the moment, evidence documented as you go, and decisions that are fair to the person and to the team

  • Run hiring for your own teams end to end, because managers own their own pipelines here

  • Proactively identify gaps in team capability, process, or priorities, and act on them without being asked

B. Own the core product surface that delivers health equity impact at safety-net organizations nationwide

  • Hold the technical direction for the messaging platform and the provider application, including reliability, performance, and incident response

  • Manage ownership boundaries and coordination between two teams working in one shared codebase

  • Set responsible-use guardrails for AI-assisted engineering across both teams, with particular care around PHI

  • Stay close enough to the systems to debug an issue, review a technical spec, or prototype an idea

  • Be familiar with the “why” behind the products you support, and push back when team output doesn't reflect health equity principles

C. Work collaboratively to deliver user-centric products that solve meaningful problems for our users

  • Connect the team's work to our mission, ensuring results are measured on impact

  • Partner with Product and Design to define and sequence the most meaningful work, often through Product Requirements Documents

  • Own delivery commitments to a date, whether that means shipping on it or renegotiating it with advance notice, never discovering the slip afterwards

  • Own the compliance posture of patient messaging alongside Compliance and Customer Success

What You Will Achieve

Within 30 days: Take over the teams and learn the product surface end to end, with a focus on understanding the user journey from a customer identifying a use case to a patient receiving a message

  • Own 1:1s and the operating cadence for both teams

  • Have a working understanding of the core application architecture and the messaging delivery pipeline end to end

  • Deliver a written read on the true state of the shared backlog: what is committed, what has stalled, and what has quietly been abandoned

  • Have a working relationship with your counterparts in Product, Design, Customer Success, Sales, and Compliance

Within 60 days: Own the release and set ownership boundaries

  • Own the delivery plan and the date for the current quarter's customer-facing release

  • Have explicit ownership boundaries between the two teams in the shared codebase

  • Have every cycle commitment either tracked to close or openly renegotiated

Within 90 days: Run both teams independently and raise the bar for Engineering

  • Run both teams' cadence without the Director in the room

  • Have given every engineer at least one documented feedback conversation and a named growth goal, with particular attention to the earlier-career engineers

  • Have shipped one improvement that changes how all of Engineering operates

Key Performance Indicators (KPIs) for this role may include:

  • Release predictability: commitments shipped on the committed date, or renegotiated before the date rather than after

  • Message delivery reliability, and how quickly delivery degradation is detected

  • Engineer growth: every engineer has a current, named growth goal and evidence of progress against it

What You Will Bring

  • You have managed engineering teams through a full performance cycle. That means hiring, consistent feedback, growing people when no promotion is available, and acting when someone is not meeting the bar for their level.

  • You have owned a customer-facing production application end to end, including reliability, on-call, and incident response, in an environment with regulated or sensitive data.

  • You have run high-volume messaging or comparable customer-communication infrastructure, including the delivery and compliance constraints that come with it.

  • You have led two teams working in one shared codebase, holding ownership boundaries without slowing either team down.

  • You stay hands-on enough to be useful. You can debug an issue, review technical work, and unblock the team without taking the work over, and you have brought AI-assisted engineering practices onto a team with responsible-use guardrails where sensitive data is involved.

  • You are comfortable with health equity work. Millions of the messages your teams send reach patients who have few other points of contact with the health system, and you will be expected to apply that lens to your team's priorities, not just endorse it.

Location

This is a remote role within the United States. Occasional travel is expected for team offsites and company gatherings.
