Position Description

We are seeking a compassionate and dedicated nurse to join our team and make a real difference in the lives of individuals living with diabetes. In this role, you will play a pivotal role in empowering clients to actively manage their condition and achieve optimal health outcomes. You will provide personalized coaching, education, and support, working collaboratively with clients, their families, and other healthcare professionals to develop and implement individualized care plans. This position reports to the Health Services Manager (Ghana).

Key Responsibilities

  • Deliver patient education sessions on various diabetes topics, tailored to individual needs and learning styles.
  • Provide ongoing coaching and support to help clients set and achieve specific treatment goals.
  • Guide and mentor clients on how to effectively manage their diabetes through lifestyle modifications, medication adherence, and self-care strategies.
  • Closely monitor clients' progress towards their goals, analyzing data and identifying areas for improvement.
  • Identify and report adverse events promptly and accurately
  • Lead initiatives to increase client enrollment and engagement in these programs.
  • Stay updated on diabetes management through ongoing education and training.
  • Prepare accurate and timely weekly, monthly, and quarterly reports on client outcomes, program effectiveness, and resource utilization.

  • Works effectively as a team member and promotes collaboration within and outside the immediate team.
  • The ability to thrive in a matrix environment which requires working with multiple teams internally (this role will liaise with our Finance, Technical and Supply Chain teams)

Qualifications

  • Bachelor’s Degree in Nursing
  • Registered Nurse (RN) license with current active standing
  • Minimum of 2 years of experience working with patients living with diabetes.
